Why good hospitals still lose money

Most distressed hospitals we assess are not short of patients or clinical talent. They are short of visibility. Nobody in the leadership team can say, on any given Monday, which service line made money last week and which one quietly consumed it.

The three most common leaks are pricing that has not been revised against cost, payer mix that has drifted towards low-yield contracts, and bed and OT utilisation that looks healthy in aggregate but collapses when you split it by day and by specialty.

The fix is unglamorous: a costed service-line P&L, a weekly operating review with the same twelve numbers every week, and one accountable owner per number. Hospitals that adopt this rhythm typically recover several margin points before any new investment is made.
